A coroner conducts investigations on the worker’s fatal death

A coroner is an official who investigates the reasons of the death of a person especially if the cause is unexpected or violent.

The coroner is investigating the death of a worker who fell from the fourth floor of the new Irving Oil Headquarters.

A man in his 30’s who is an insulator for Grandview Insulation Contractor Inc. was working on the fourth floor of the new Irving Oil Headquarters insulating a heating pipe when he fell to the ground. According to a witness who also worked in the building, he heard a sound like a ladder falling on the ground. And when he looked down, he saw the insulator man lying on the ground a distance away. The new Irving Oil Headquarter is an 11-storey building when it is completed and will house 1000 Irving Oil employees.

The arm or wrist of the man might be broken, and the man started to convulse and rolled over on his stomach. The man was taken to the regional hospital and eventually died due to injuries on the head caused by the fall.

The coroner started investigating the cause of the death. The chief of the coroner presides over the investigation; the prosecutor, acts as council to the coroner and the lawyer for the construction services company. Witness started to be investigated.

According to the prosecutor, there is a safety practice at the Irving area, that in the event of an accident, workers have to go to the first aid station and sound three blasts on an air horn to call the in-house first aid team, and this practice did not happened.

The other workers tried to stabilize the seizing body of the man and called emergency numbers. Another person volunteered and took off his sweater and placed it under head of the victim where there is already plenty of blood coming out of the head.

After the death of the worker, the foreman or the supervisor of the building ordered eight scissor lifts, or an elevating platform instead of ladders to be used by insulators and to minimize accidents.

According to the health and safety co-coordinator, in the area, requires all workers to take “fall arrest training” to lessen the impact of accidents. The workers should use hard hats with straps, they are especially designed to resist impacts and keep the hats in place in a fall.
